This Waiver will remain in effect whenever you participate in The Makery Lab by using The Makery Lab facility, tools, and equipment. Failure to meet the requirements of the Waiver may result in the ban of a person from participating in The Makery Lab.

I understand and acknowledge that there are risks of personal injury and property damage while participating in activities using The Makery Lab facility, tools, and equipment (collectively The Makery Lab) offered by The City of Chandler and Chandler Public Library. I understand that some risks are inherent in the use of The Makery Lab and that some other risks may arise from conditions, situations, or activities that I engage in or create at The Makery Lab. With full understanding of the risks involved and despite this Waiver, I am willing to participate and use The Makery Lab.

I, therefore, agree I will not use any equipment in The Makery Lab until I determine I can operate it safely and properly, and that I will abide by all rules. While The Makery Lab is in my use, operation, or possession, I agree to be responsible for maintaining the facility, tools, and equipment in good working order. I agree that I will be responsible for any repair and replacement costs due to negligent or intentional damage or loss that occurs to equipment. I understand that certain skills and safety procedures are required to mitigate the dangers involved in using The Makery Lab.

I agree to assume the risk that unexpected events may occur and result in harm, injury, or illness to me or damage to my property while I am participating in, or observing activities or using The Makery Lab.

I agree to waive and release all claims that I have or may have in the future against the City, its officers, employees, volunteers, committees, and boards from any and all liability and negligence for any damage or injury that may occur, regardless of the cause, resulting from my participation in The Makery Lab including, but not limited to, the use of any tools or equipment available for use as a part of The Makery Lab.

I agree that I will not create any weapons using The Makery Lab. Participants in The Makery Lab are prohibited from using equipment to manufacture, develop, or create any object that could be deemed a weapon (defined as an object capable of being used to inflict great bodily injury or death).

I recognize that signing this Waiver means I am giving up, among other things, rights to sue The City of Chandler for injuries, damages, or losses I may incur while using The Makery Lab. I also understand that this release binds my heirs, executors, administrators, and assigns, as well as myself.

I certify I have read the entire Waiver, and I fully understand it and agree to be legally bound by it as an express condition to my use of The Makery Lab.

I agree that The Makery Lab may only be used by a person who is at least sixteen (16) years of age. If a participant in The Makery Lab is under the age of eighteen (18), their parent or guardian is required to sign the Waiver and be present during the use of the Makery Lab.
