Student Activity/Event Approval Form

This form is the first step in planning any activity or event including but not limited to:

  • social gatherings
  • travel (academic and student organization)
  • community service projects
  • on campus events


These activities/events require prior approval from the Director of Student Life and Development and/or the Dean of Student Affairs at least 10 business days prior to the start date.


Activity/event sponsors will receive an approval/denial notification via email. This approval is needed before any reservations or purchases are made.
