YES. I agree to obtain proper parking permit(s) from UA Parking & Transportation Services prior to the scheduled date of operation to be displayed in the food truck windshield on the date of operation. The University of Arizona required vendor parking permit cost is $12 per day. If I am chosen to be part of the rotation, I agree to immediately contact Jen Bomesberger (jennyb@arizona.edu) at UA Parking & Transportation Services two weeks prior to my shift.
I agree to comply with all campus, county, city, state, and federal safety requirements.
If you do not attach any of the following required files, we will follow-up with you to collect these documents. Your application will be moved to approval consideration once all documents have been submitted.

What are the dimensions of your Food Truck?
Commercial General Liability Coverage: Minimum $1 million/$2 million aggregate.
Commercial Auto Liability Coverage: Minimum $1 million. This requirement applies to self contained food trucks AND the vehicles that pull food trailers. A General Liability Policy covers your business, but does NOT cover Auto-Related Accidents.
Workman’s Compensation Coverage: $100,000; Only applicable for food trucks with employees, not independent contractors or hired day labor.
Additional insured/certificate holder names to be on all policies as the following and the Additional Insured Box (”ADDL INSR”) must be checked: Arizona Board of Regents, State of Arizona & The University of Arizona.

Please email your Certificates of Insurance to su-vendors@arizona.edu within 5 days of filling out this application.
The Additional Insured Box (“ADDL INSR”) must be checked.
On the bottom half of the form in the Description of Operations box, please have your agent place the following text in the box: The State of Arizona, the Arizona Board of Regents, and the University of Arizona has been listed as additional insureds with waiver of subrogation.

I guarantee my use of a quiet generator during each scheduled date of operation
I guarantee my use of an electronic point of service system with financial reporting capabilities for all customer transactions.
I guarantee my ability to accept cash, credit/debit cards, and digital forms of payment.
I guarantee my use of wooden boards to place underneath all tires of my food truck.
I agree to provide a trash can for customer waste disposal outside of the food truck and remove all trash and any related debris from the premises on each scheduled date of operation.
I agree to abide by all University of Arizona policies and procedures as directed.
