This form is to be used in accordance with ICJ Administrative Policy 01-2021: Juvenile Record Expungement and the expungement polices and laws of the state seeking expungement. Commissioners, Compact Administrators, Designees, and Deputy Compact Administrators from an adjudicating or home/demanding state are responsible for submitting expungement requests to the National Office.
