Home Rehab Pre-Screening and Referral Tool
Effective June 10, 2025, the Travis County CDBG Office will use this pre-screening tool to determine if a potential home rehabilitation client meets certain minimum criteria for the County's Home Rehab Program, including but not limited to the following:
- Service Area Eligibility - must live in the Travis County CDBG jurisdiction.
- Homeownership - must be a homeowner and must occupy the home as your primary residence.
- Income Eligibility - must convey information that indicates income eligibility (will be verified by program administrator).
- Floodplain Clearance - home must not be in a 100-year floodplain area.
The County's program is administered by Meals on Wheels Central Texas, who established a subrecipient agreement with the Travis County CDBG Office. If a resident meets minimum criteria on this pre-screening tool, Travis County CDBG staff will refer the resident to Meals on Wheels to complete the application, at which time an official determination of program eligibility will be made.
Completing the pre-screening does not guarantee service or program eligibility. It gives a precursory indication of possible home rehab program eligibility. CDBG staff also aims to answer questions and help each applicant prepare for the full application. Additionally, the information provided may help County staff determine how best to serve an applicant. If a staff member completes the pre-screening with an applicant in real-time, a determination will be made the same day. If an applicant is completing this form individually, please allow up to 30-days for a follow-up. A response of eligibility or ineligibility will be provided in writing. If an applicant is determined to be eligible, based upon the pre-screening criteria alone, the applicant (homeowner) will be invited to complete the full application with Meals on Wheels Central Texas.
