Veteran’s Memorial Commemorative Brick Order Form

Honor a Veteran with an engraved brick as a permanent tribute for serving in the Armed Forces of the United States.

Cost: $150.00 per brick

For Memorial Day dedication, orders must be placed by April 1.

For Veteran’s Day dedication, orders must be placed by October 1.

Brick Inscription

Each line of the brick is limited to 21 characters including letters, numbers, punctuation, and spaces.

Please look at the examples below for sample inscriptions and character counts.

Line 1: Name on brick – First Name Last Name (21 character limit, including spaces)

Type the name of the veteran as you want it listed on the brick.

Ex 1. Kenneth Wilson (14 Characters)

Ex 2. MSG Kenneth Wilson (18 Characters)

Line 2: Branch or Rank or Unit (21 character limit, including spaces)

Use this line to note the branch of service or rank or unit or any other notation that is relevant and meaningful.

Ex 1. USAF Purple Heart (16 characters)

Ex 2. USMC 1958-1968 (14 Characters)

Line 3: Service Location, War Service, Years of Service (21 character limit, including spaces)

Ex 1. WWII–France 1944-1945 (21 Characters)

Ex 2. Desert Storm 1991-1992 (22 Characters, this will not work)
