This form can be submitted by submitted by Directors of Graduate Programs (DGP) or Graduate Services Coordinators (GSC) only.

Please complete the Graduate Student Support Plan (GSSP) Exception Request form for review and approval by the Graduate School. Exception requests are evaluated for one semester at a time only. Students should have a completed and approved Graduate Plan of Work (GPOW) or Degree Audit before any request is evaluated.

You are encouraged to attach supporting documentation, but please do not upload specific medical information so that we can remain in compliance with HIPAA.

Submission Deadline: Friday before Census Day each semester

We will begin reviewing Fall 2025 exception requests in mid-May
