Member Database Request Form
The Member Database Request Form is an internal form where NACDD staff and consultants can submit requests to Member Services. Contact Members@chronicdisease.org with questions or to request additional support.
You can submit one entry per individual, or you can submit a bulk entry that includes multiple individuals:
- To submit one individual per entry, choose options 1, 2, or 3 below.
- To submit a bulk request with multiple individuals in one entry, select “Multiple requests” in the “Type of Request” dropdown.
Examples of requests include:
1. Database / Engage requests:
- Adding, removing, or editing records in the Member Portal
- Adding Members to a specific Engage community
- Creating a new Engage community
2. Communication Preferences requests:
- Adding or removing Members to NACDD newsletters (i.e. Mailchimp groups)
- Adding or removing Members to Invite-Only Communications (i.e. Mailchimp tags)
3. Both:
- This means your request is both a Database / Engage request and a Communication Preferences request
4. Multiple Requests:
- This option allows you to upload a spreadsheet with all your requests included in one submission.
5. Report Request:
- Select this option if you need Member Services to pull a specific report about Membership from our Member Portal.

provided by Member Services OR ensure that your template includes all the information that Member Services needs to fully understand your request and take the appropriate action(s).
Timeline:
We aim to process requests within 5-7 business days. However, there are instances when we may need more time due to other priorities or the volume of the request(s). We will contact you if we cannot fulfill your request within this timeline.
