Specialty Equipment Insurance - Enrollment Form

Use this form to request coverage for state-owned, high-value, or specialty equipment under Mississippi State University’s optional Scheduled Property Floater policy. This program provides supplemental coverage for specific equipment and may reduce the standard $100,000 departmental deductible in the event of a covered loss.
