New Dog License Application

The City of South Jordan requires dogs to be licensed. To apply for a new dog license, please complete the following form. After submitting your application, it will be processed and a bill mailed with payment instructions. Once payment is received, you will be mailed a tag.


Fees:


Dog License (altered) - $10/annually

Dog License (unaltered) - $35/annually

Dog License, Senior Citizen – Age 65+ (dog must be altered)* - $5/annually

*Seniors must present their ID in person at City Hall the first time they are charged the senior discount rate. Please do not fill out this application if seeking the senior discount rate.


Per the City of South Jordan's Municipal Code:


  1. Homeowners, renters, or other insurance shall cover any dog required to be licensed.
  2. No person shall own or harbor more than two (2) household pets per species over the age of four months per dwelling.
  3. The City shall issue a license for a dog located in the city upon satisfaction of the following conditions:
  4. Owner of the dog completes an approved application.
  5. Owner of the dog pays the required fee (fee set by resolution of the City Council)
  6. Proof of updated rabies vaccination or a notarized letter from a veterinarian is provided stating a dog cannot be vaccinated due to health reasons.
  7. The owner of a dog shall at all times cause the dog to wear the license tag.
  8. Please refer to City Code Title 6 for all ordinances and requirements for licensing/owning a dog in the City of South Jordan.
