Please use this form to request changing your home address for GPS monitoring. Once the form is submitted, an OSPS member will reach out to you.

Please contact your Pretrial Service Officer in addition to submitting this request.

This form is only a request. The request is not approved until you receive further contact and confirmation from OSPS of approval.

Leaving your inclusion zone or changing your address without approval can result in a violation notice to the court and may result in increased supervision requirements or pretrial detention.
