This form is used to collect information for all visitors to UT Physics including students and visiting researchers/scholars.

Visiting students will need to fill out all sections provided when selecting "Visiting Student".

Visiting researchers/scholars will need additional materials prior to submission;

1. You will need to have the following documents available for upload:
2. CV (Word of or Adobe PDF).
3. A clear scan of your valid government ID (passport, driver's license, etc)

This survey is used to request central approval for a new or renewing nonemployee research affiliate appointment for an external individual to work or collaborate in UT Austin facilities.
