Field Trip Registration Form

All OCPS sponsored field trip events MUST be registered with Risk Management.


Day and overnight field trips MUST be registered with Risk Management 30 days prior to date of trip.


International field trips MUST be registered with Risk Management 90 days prior to date of trip.


Complete all applicable fields relevant to your field trip.

All fields with an (*) are required before you can proceed to the next section.

 


The Field Trip Coordinator is responsible for registering all field trips through the online registration portal and should be the only person submitting all field trips.


The Field Trip Coordinator is the main point of contact for all field trip related items and correspondence between the school and the Risk Management Department and Principal Leader's office.
