Our diverse lived experiences are critical to shaping healthcare policies, programs, and services that truly meet the needs of patients and their families. Authentic partnerships with Patient Family Partners (PFPs) are at the cornerstone of our mission.

All Patient Family Partner experience levels are welcome. You will have the opportunity to meet other Patient Family Partners, observe partnership in action, and become part of a community. The purpose of this Interest Questionnaire is for us to understand more about you so that we can build programs to meet your needs. This information will also help so that we can match you with meaningful partnership opportunities, if you wish to participate. We are so glad you’re here!
