Accommodation Request
The Fashion Institute of Technology (“FIT”), in accordance with the Americans with Disabilities Act of 1990, and the ADA Amendments Act of 2008 (“ADA”), Title VII of the Civil Rights Act (Religious Accommodation), and the New York State Human Rights Law (“NYSHRL”), New York City Human Rights Law (“NYCHRL”), and Pregnant Workers Fairness Act ("PWFA") of 2024, will, in good faith, engage in a cooperative dialogue with qualified employees or applicants who request a reasonable accommodation under certain circumstances. The process of determining whether, and what type, of reasonable accommodation should be granted is an interactive process between FIT and the requesting individual and may require additional information and documentation to respond to a request. If you are experiencing difficulties and are unable to work due to a serious medical condition, leave of absence options may be available.
The information submitted via this form is confidential and is only available to those FIT employees who need to know to fulfill the obligations of the ADA, NYSHRL, NYCHRL, PWFA, Title VII, or other leave-related law or regulation, policy, program, or administer the provisions of the UCE collective bargaining agreement.
Complete the following information and HR will follow up directly to discuss your request and additional documentation that may be needed.
