Provider Grant Application

**Thank you for your interest in our Provider Grant Program. Our Provider Grants are closed for the remainder of the month- the application will reopen on July 1st.


Additional innovation and stability funding will open after July 1, 2023 in the new fiscal year. Email communitysupports@dpcolo.org with any questions.


Provider Grants are intended to increase provider capacity to serve individuals with intellectual or developmental disabilities or delays (IDDs) in Adams, Arapahoe, Douglas, and Elbert Counties. Grant funds may also stabilize programming and provide supports to increase staff capacity.

Note that an Organization Budget and Project Budget must be submitted to complete the application and to move forward in the review process.

Note on Project Budget

If not attached with application project budget must be submitted within 14 days to ensure review. Otherwise, application will be withdrawn.

Upload the following:


1. Annual Budget/Year End Financial Statement 2. Balance Sheet (List of assets and liabilities) 3. Project Budget 4. Other Supporting Documentation (Project Plan, Gantt Chart, Letters of Support, Estimates/Price Quotes, etc.)


Note: A business plan is required for new organizations.
