Special Education Facilitation Request Form

Please fill out this form to request Special Education Facilitation Services.

 

Optimally, we prefer a two week lead time for requests to arrange facilitation services.

Questions about these services can be directed to Katherine Rains, Supervisor of the Dispute Prevention and Assisted Resolution Team, at 720.990.1464 or rains_k@cde.state.co.us. On occasion, she may ask to attend any of these meetings for purposes of evaluating the service. Parties will be notified ahead of time, and any questions will be addressed at that time.

Please read through the information below prior to completing and submitting the request form below.

 

Special Education facilitation is a voluntary process;

Both the parent and the district need to agree to the facilitation. Special Education facilitation is an option for using an impartial third party to promote effective communication and assist the IEP team in developing an IEP based on the student's needs. Facilitators are also available for Special Education meetings other than IEP meetings.

 

Rights & Responsibilities

Special Education facilitation does not relieve the district of the responsibility to meet regulatory obligations, including timelines. If an agreement is reached on the IEP, the school district is required to complete the IEP document and provide a copy to the parent or adult student, as well as send notice regarding the components of the IEP. The only record kept of the facilitated session includes the date, time and location of the session, surveys and the result. Neither the ESSU nor the facilitator will keep the IEP document.
