Accessibility Request Form

Players, caddies, and spectators with permanent physical disabilities can request motorized or mechanical transportation (golf cart) at any Golf Ontario competition by submitting an Accessibility Request Form.

Request Deadline: All Requests must be submitted 3 days prior to the request date.

Please Note: Accessibility Requests are subject to cart availability and host course rules.

To support your request, please attach a recent letter from your physician that includes the following:

i)    a brief description of your physical disability;

ii)    confirmation that the physical impairment is permanent;

Approval Process: Accessibility requests will be reviewed by Golf Ontario, and upon acceptance, cart availability/eligibility will be requested from the host course. If this application is approved, you will be required to sign a ‘Conditions of Use Form’, which will outline the guidelines for use during the championship. Failure to abide by the guidelines may result in a forfeiture of immediate and future Accessibility accommodations.

Approved Accessible Transportation requests must adhere to the following;

Must meet the club criteria for use (i.e., minimum age, driver’s license).

The cart must be returned to the pro shop immediately following completion of the golf round for the day.

The cart must stay out of any fescue grass areas.

The cart must NOT provide transportation for other players, caddies, players' equipment or spectators at any time.

The cart must follow the Golf Club’s guidelines for Cart Use at all times.

Spectators Additional Conditions

• Keep carts on the path at all times. If cart paths are not available, carts must be kept out of play near the perimeter of the hole.
• Must follow ONE (1) group of players on the course during the competition.
• Stay with the cart at all times.
• Be courteous to all other groups on the golf course
• Follow any and all of the Local Cart Rules published by the host facility
• Allow any players or caddies sit or ride in the cart for any reason
• Allow any other person to sit or ride in the cart for any reason
• Drive the cart while looking for golf balls

I agree to abide by the Conditions of Use, and I understand that if I do not abide by the Conditions, the permission to use the cart may be rescinded.
