Summer Programs: Student Information


Berklee College of Music, Delegación de la Fundación en España (hereinafter, “Berklee Valencia”) needs the following information for administration and emergency purposes during the Summer Program you will be attending. Complete this form as soon as possible.


Please note that if the student is a minor -according to their national law-, this form shall be entirely filled out and accepted by their parent authority/legal guardian.

Parent / Legal Guardian (If student is a minor)

The following information shall only be provided if the student is a minor (under 18 years of age, in any case, or still a minor, according to their National Law), by their parent authority/legal guardian.


You shall be aware that Berklee Valencia, will not take on any responsibility for students outside of school hours of the Summer Program as specified in the brochure thereof Summer Program.


Therefore, the parent authority/legal guardian shall adopt the appropriate measures to ensure that the student receives the necessary care, supervision and assistance outside of school hours.

Should an emergency situation occur or should the student get seriously hurt or suffer a serious disorder or disease during their participation in the Summer Program which might prevent the student from taking conscious decisions, and / or should the student be considered missing; I hereby authorize both Berklee College of Music Inc. and its branch Berklee College of Music Delegación de la Fundación en España and their staff; to notify and inform the individual listed on the Emergency Contact form below about the emergency situation occurred.

Acquire Travel Insurance

All students enrolled in one of the Berklee Valencia Summer Programs (BV, Camp, Music Production and Summer Performance Program) will be required to show proof of a health insurance policy for the duration of their studies at Berklee prior to the start of the program. Please note that:




  • Students who are not nationals of an EU member state can upload a copy of an adequate and valid travel medical insurance policy which complies with EU regulations, to cover any expenses which might arise in connection with repatriation for medical reasons, urgent medical attention and/or emergency hospital treatment or death, during their stay. (You may use this information posted on the official website of the Ministerio de Asuntos Exteriores, Unión Europea y Cooperación (Consulate of Washington) as a reference to indicate what would be in compliance with EU regulations.)

Legal Terms

The controller in charge of processing your personal data

Purpose of processing

Berklee will collect and process your personal data for the management of the onboarding and administrative processes and, where appropriate, the subsequent execution of the pre-contractual relationship with Berklee. Also, in extraordinary circumstances, emergency contact information may be needed as part of the emergency management plans within the institution.


The legal basis for processing

Your data will be processed pursuant to your consent.


You have the right to withdraw consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal. Interested parties who do not allow their personal data to be processed may not be able to join any academic program in Berklee.


Communication of personal data

Third parties will only have access to the personal data if Berklee is legally required by law, or when it is strictly necessary to execute a service contract between Berklee and third parties. Retention of personal data that is provided through the present form will be processed during the time the Summer Program is active. Once the Summer Program is terminated the data will be duly blocked so long as legal liabilities may derive for the party. It is your responsibility to provide reliable data to Berklee and keep this information updated.


How interested parties may exercise their rights

Pursuant to the applicable legislation, Berklee is committed to respecting the confidentiality of its users’ personal data and to ensuring that interested parties may exercise, at any time and in the terms set forth by the legislation in force.
