As part of the Oregon Health Plan’s (OHP or Medicaid) Health Related Social Needs (HRSN) benefit to offer climate devices and services, nutrition services and housing supports to members experiencing a qualifying life transition, EOCCO will be granting Community Building Funds (CCBF) to eligible community partners.

CCBF is designed to help organizations build infrastructure to support the new Health Related Social Needs benefit, which provides climate devices, housing, and nutrition support to eligible members. Community Capacity Building Funds can be used to support four categories of work:

1. Technology
2. Development of Business or Operational Practices
3. Workforce development and;
4. Outreach, education, and convening
