Public Safety Advisory Bodies Joint Application

Personal Identifying Information

Applications to public bodies are subject to disclosure under Oregon’s public records law. This includes, in some circumstances, personal contact information such as residential address, personal email addresses, and personal telephone numbers (“personal contact information”).

The law applies differently to applicants who are selected for positions and applicants who are not selected for positions. If an applicant is selected for a position, their personal information may be redacted by law. If an applicant is not selected for a position, the personal contact information contained in the application is subject to disclosure. Please select from the options below before completing your answers in this section:

1. I do not want my personal contact information to be disclosed. I am submitting this information on the condition that it is kept confidential, and I personally consider this information confidential. Without the promise of confidentiality, I would not have applied for this position.

I understand that I am voluntarily completing this application, and I understand that the City will make every effort to keep the following information confidential. I understand that redactions and exemptions are at the discretion of the City and while the City will attempt to keep such information confidential, the City may be required to turn it over or may inadvertently disclose the information and thus confidentiality cannot be guaranteed.

2. The City may disclose my personal contact information to the extent allowable by law.

Conflict of Interest Questions

City advisory bodies provide critical advice to improve the City’s performance on public safety, and community confidence in the City’s public safety responses. Members are placed in a position of trust and become “public officials” (at least while performing the volunteer work) under the law.

Please help us by answering the following questions based on state ethics standards. If you are unsure or uncertain, please describe your situation. That information is helpful to us, and we can follow up if we have further questions. Failure to disclose a significant conflict of interest could mean the City must remove you from the board.
