Student Equipment Request Form

Filling out this form does not mean that equipment is available or will be provided. You will need to see the representative at the Ingram Library circulation desk to check availability. Be sure to check the Ingram Library Hours before you head there to make sure they are open. (https://www.westga.edu/library/index.php)

These items are due on the date determined by the Ingram Library (5/19/25 for Spring 2025). Be sure to write that date down and keep it for your records. If items are not returned on time, a hold will be placed on your student account, fines/replacement costs will be applied for lost or damaged items, and you may not be able to check items out in the future. Also, failure to return items in the condition in which they were checked out could result in holds, replacement fees, etc., and could result in the student not being allowed to check out items in the future. Limited numbers of equipment are available and requests could be denied. Please describe the impact a request denial would have on your studies in as much detail as possible, making sure to remember the availability of on campus computer labs.

You will not not be able to load software onto the computer yourself. If you need additional software on the laptop, first please check the UWG Virtual Lab (https://www.westga.edu/its/virtual-computer-lab/) to see if you can access the software that way. If you cannot, contact ITS at 678-839-6587. List any needed software below.
