University of West Georgia Major, Minor and Stand-Alone Certificate Declaration Form

Undergraduate degree-seeking students may use this form to request the following changes to their program of study. •Request to change/declare a major or secondary major •Request to change/declare a minor or secondary minor •Request to change/declare a stand-alone certificate program •Request to remove a program of study (major, minor, stand-alone certificate) Prior to submitting a request to change their major, students should consult their academic advisor for assistance. Requests are typically processed within 72 business hours of submission. You will receive an email when your request is complete. If you have any questions or concerns, please contact the UWG Graduation Team at graduation@westga.edu.

Major, Minor or Certificate Declaration Agreement

Before proceeding, please read this carefully. Any change made in a student's program of study, especially if made late in the student's college career, may require additional courses beyond the 120 hours typically required for a bachelor's degree. Depending on your chosen major, you may be required to take additional courses in the core or take additional courses as pre-requisites to major courses. Prior to submitting a request to change your major, it is highly recommended that you meet with an academic advisor to discuss changes in academic requirements. You may complete a What-If Analysis in Wolf Watch to see how your current courses will align with a new major. If you are receiving financial aid, it is also recommended to meet with a financial aid advisor to discuss how a change in major may affect your financial aid. Students may visit the Momentum Center to meet with an academic advisor and discuss potential issues with their financial aid. Per the UWG Academic Catalog, it is the responsibility of each student to keep himself or herself apprised of current graduation requirements for a degree program in which he or she is enrolled.

BFA Theatre Program Agreement*

The Bachelor of Fine Arts in Theatre degree is considered a competitive program and requires review by faculty for admission. Considering this, you will be required to declare your major as a Bachelor of Arts in Theatre while you are working towards completing the admission requirements for the BFA program. After completing this form, you will be contacted by faculty in the Theatre major with more information about the BFA program.

BFA Art Program Agreement*

The Bachelor of Fine Arts in Art degree is considered a competitive program and requires review by faculty for admission. Considering this, you will be required to declare your major as a Bachelor of Fine Arts with a Concentration in General Fine Arts while you are working towards completing the admission requirements for your chosen BFA concentration. After completing this form, you will be contacted by faculty in the Art major with more information about the BFA program.

College of Education Program Agreement*

For Elementary Education, Special Education, Physical Education and English/History - Secondary Education Majors: Acceptance to the teacher education program requires additional review, testing, and prerequisites. You will receive an email from a College of Education advisor with information for scheduling an advisement appointment. You can learn more about the Teacher Education admissions here: https://www.westga.edu/academics/education/advisement/admission-teacher-education.php For Sport Management, Health & Community Wellness, and Speech-Language Pathology Majors: Acceptance into major courses requires additional review.

Music Program Agreement*

The Bachelor of Music degree is considered a competitive program and requires review by faculty for admission. Considering this, you will be required to declare your major as Focus Area - Humanities while you are working towards completing the admission requirements for the Bachelor of Music program. After completing this form, you will be contacted by faculty in the Music major with more information about the Bachelor of Music program.

Richards College of Business BBA Majors

This program is located in the Richards College of Business. By submitting this application you agree that you have read and understand that Richards College of Business (BBA) students must successfully complete ENGL 1101 (with a C or higher), MATH 1111 (with a C or higher), MATH 1413, ECON 2105 or 2106, ACCT 2101, 45 credit hours, and have a 2.0 GPA to be eligible to take 3000/4000 level business classes. A 2.0 GPA is required for all business courses. A 2.0 GPA is required for Area F, Business Core Area, and Major Area (only one [1] D allowed in major area).

School of Communication, Film, and Media Supplemental Information

Mass Communications majors are required to complete a minor to earn a Bachelor of Science degree. Mass Communications majors are encouraged to review the list of recommended minors with their advisor prior to declaring their minor. Mass Communications majors may not minor in Communication Studies, Film & Video Production, or Mass Communications. Film & Video Production majors are required to declare a minor or complete the Georgia Film Academy Certification to earn a Bachelor of Science degree. Film & Video Production majors may not minor in Communication Studies, Film & Video Production, or Mass Communications.
