- Official Spring League Roster

• Your first official roster given to you by your club registrar for your spring league season.

- US Youth Soccer Medical Waivers

• Must be current but does not need to be notarized
• We do NOT accept US Club waivers.

- Proof of Age Documents (Accepted Documents below)

• Birth Certificate
• Passport
• Current Drivers License
• Uniformed Service ID & Privilege Card
• Alien Registration Card
• Unexpired Federal, State, or Local Govt. ID Card

- Coaching Staff Licenses Documentation

• All coaches listed on a Kansas State Cup Roster MUST have a minimum of a US Soccer D Licence
• Please provide all coaches that may coach the team to submit their coaching license for verification
• If a coach does not have or refuses to upload documentation by the time of the final day to check in then they will be ineligible to participate in Kansas State Cup. NO EXCEPTIONS!

- The Alliance for All Form

• Signed by Head Coach or Manager certifying that all parents, players & coaches have agree to show good sportsmanship protocol.

- Hotel Form (If applicable)

- PLEASE TITLE EACH UPLOAD WITH THE TYPE OF DOCUMENTS THAT ARE INCLUDED.

Please list the amount of t-shirt sizes your team (players, coaches & team managers) would need so we may tell our merchandise vendor to bring an appropriate amount of gear. As well as all champions will get a t-shirt and we base this entry on what we give you.
