Complete this form to make a referral to the Day 1 Families Fund. The purpose of these funds is to quickly rehouse families. A case manager referring a household should be actively having housing problem solving conversations with the household and provide ongoing case management, as needed and identified by the household.

To be eligible for funds, a household must:

1. have at least one adult and one dependent child
2. be experiencing homelessness
